Based on this comic and the Final story arc of 1.0, I suspect that Ethan didn’t become stupid again; he chose to make himself look like he did to make sure his friends would become sane. At the very least, he got a reasonably substantial net gain in intellect from this event. Of course, it may just be a matter of thinking things through; Ethan does have the capabilities required to independently create sentience.
Check it again. The doctor actually said he didn’t think he could legally use the word “normal” in discussing Ethan’s brain. What he did say was that the swelling would likely go down. It was Ethan who speculated he would then be back to normal. What was normal for him before that is, not normal normal.

The doctor a few pages ago literally said he would get back to “normal” since it was a few days concussion, did you just miss out that piece of story?
